CHOICE PROTECTION PLAN

Breakdowns in your home telephone inside wiring and jacks or TV wiring and outlets can be an unforeseen hassle. As the subscriber the choice of dealing with these unexpected disturbances or allowing South Slope to address them is yours. This was mandated back in the early 90’s by Federal legislation, not your local service provider. South Slope’s optional Choice Protection plan covers diagnosis and repair of the inside telephone wire and jacks that provide your South Slope local line/dial-tone service and/or inside coaxial/ethernet cable and outlets that provide broadband applications such as Video Plus TV and/or High Speed Internet service. With Choice Protection, you will not pay any labor or material charges if you need a South Slope technician to make repairs covered by the plan. Choice Protection covers your existing telephone wiring, Fiber to the Premise or DSL applications such as High Speed Internet wiring and Video Plus TV wiring.

Local Telephone Line

As a South Slope customer, you are responsible for maintaining the inside telephone wiring from the protector (usually a small gray box on the outside of your house) to your telephone devices. The transmission facilities used to deliver your service from the central telephone office to the protector are maintained by South Slope. “Inside wire” refers to the wiring running from the protector to the telephone devices in your home. Customer owned interface equipment including but not limited to; Cordless and hardwired telephones, line splitters, FAX machines, modems, satellite receivers or answering machines that may be connected to a telephone jack are not included in the definition of “inside wire.”


 

Broadband Applications

As previously mentioned, you as a South Slope customer are responsible for maintaining the inside cable/ethernet cable from the Network Interface Device (NID) or protector to your broadband device. The transmission facilities used to deliver the signal from the broadband application to your home is maintained by South Slope. Customer-owned electronic systems or entertainment devices such as televisions, hubs, switches, routers, VCR/DVD players, satellite receivers, are not included in the definition of “inside wire.”

Customers who choose NOT to subscribe to the Choice Protection plan are responsible for the maintenance of their inside wiring. You have three options to repair faulty inside wiring: make the repair yourself, or others of your choice, or contact South Slope to make the repair based on an hourly rate. (A one hour minimum service call will be assessed on all dispatched calls.) Currently, the minimum charge for service calls is $50 an hour plus material costs.


 

Pricing and Changes

The monthly charge for Choice Protection is $4.95 plus applicable taxes. South Slope may change any of the terms and conditions and increase the monthly charge for the Plan. When significant changes in the terms and conditions or increases to the monthly charge occur, we will notify you 30 days prior to the effective date of the change or increase and if applicable, new Terms and Conditions will be sent to you.  The notice may be included with your monthly bill and your continued use or payment for the plan after such a change will be acknowledgement of your acceptance of listed changes.


 

 Terms and Conditions

1. Under the Choice Protection plan, South Slope will repair telephone service problems which are in the wiring and jacks inside your residential home or single line business. If the problem is in the wiring between the protector and the jack, or in the jack, we will repair or replace the wire and/or modular jack. If the problem is caused by defective or faulty telephone equipment, South Slope will let you know the source of the problem. South Slope will repair work in a reasonable manner, so we may reroute wire along baseboards or some other location to avoid replacement or repair of drywall, plaster or other materials and to avoid unnecessary labor and cost.

Under the Choice Protection plan, in addition to the terms and conditions listed in Item One, South Slope will repair broadband issues which are in the cable/ethernet wiring and outlets inside your home or business. If the problem is in the wiring between the NID/protector and the outlet, or the outlet and the South Slope provided broadband device, South Slope will repair or replace the defective cable and/or outlet. South Slope will repair or replace defective splitters. Installation of additional broadband devices may warrant the customer’s necessity to purchase additional equipment such as line amplification equipment or splitters. 

2.                    3. You agree to pay a monthly charge for Choice Protection. There is no added charge to repair problems in the wiring, jacks or outlets inside your home or business. Choice Protection is provided on a month-to-month basis and may be canceled by the customer or South Slope at any time by giving verbal or written notice to the other. Choice Protection will cover residential and single business lines.

Choice Protection does not cover (1) problems caused by willful damage to the facilities used in the transmission of telecommunications services or inside wiring, jacks and outlets; (2) inside wiring that is not working or disconnected at the time Choice Protection coverage is initiated; (3) the activation and re-activation of jacks or outlets, relocation of jacks or outlets, replacement or installation of new inside wire, jacks or outlets; (4) Damage which results from fire, earthquake, flooding, natural disasters, vandalism, intentional abuse and the like or other Acts of God; (5) Lines with a telephone key system such as a PBX or other non-basic telephone system, distribution panels or other non-wire parts of so-called structured wire systems; (6) Customer owned wireless interface devices; (7) Choice Protection does not cover buried or underground cable or wiring that runs between or among separate building, apartments or dwelling units in a multi-tenant property; (8) inside wire, jacks or outlets that do not meet industry standard for telecommunications; and (9) any repair or replacement that would endanger the safety of a South Slope technician. “PREMISE INELIGIBLE” shall be used to define those individual premises determined by a South Slope Technician, in his or her discretion, to contain non-standard, ineffectual or faulty inside wiring.  If a premise is identified as such, Choice Protection service shall be discontinued immediately, and absolutely no repair shall be conducted on these premises under this contract.
