ESTABLISHING SERVICE/CHANGES
What will the Telephone Company need to establish service?

  1. Complete street address including apartment number if applicable.
  2. Information on previous telephone service - where, when and the telephone number.
  3. Social security number for residential service.
  4. Business ownership information for business service.
  5. Employment and reference information
  6. Choice of long distance companies for inside and outside your area code.
  7. Choice of directory listing.
  8. Choice of products and services desired.

HOW DO I ESTABLISH CREDIT AND WILL I BE ASKED FOR A DEPOSIT?

Credit history is a customer's record of monthly payments for service provided by a telephone company. Deposits are requested from all applicants without a credit history or upon evaluation of the customer's credit which would equal 2 months of local service and installation costs. An additional deposit may be required for Long Distance Service.

WHEN WILL MY DEPOSIT BE RETURNED?

Every deposit (with interest) is refunded after 12 consecutive full monthly payments have been received. If not previously refunded, each deposit is applied as a credit on the account when service is disconnected. If a credit balance results, a check will be issued within 60 days.

HOW WILL I BE LISTED IN THE PHONE BOOK™

You may choose how you want to be listed. A listing with your complete street address is free in this directory. Additional listings are available for a small monthly fee. However, if you don't want your name in this book, you have other choices. Contact the Customer Care Center for details.

CHANGING LONG DISTANCE CARRIERS

You may change your carrier by writing a note with the phone numbers you want switched, your choice of carrier, the date you would like switch and your signature. Make sure the long distance carrier understands that you will handle the switching by notifying your local phone company. FCC rules require we charge customers to change carriers, although it is common practice for long distance carriers to reimburse you for this cost. If you have a question about long distance, please call our office and we will try to help.

HOW DO I DISCONNECT MY SERVICE?

Notify the Customer Care Center of the date you want your service disconnected. Provide a forwarding address for the final bill.
